摘要

目的

总结1例阴道分娩后发生急性炎性水肿性外痔产妇的中医护理经验。

方法

辨证采用底轮灸配合输液式中药湿敷作为核心干预方案，同时配合膳食指导、镇痛管理、情志护理及生活指导等综合措施。

结果

经干预后，患者症状显著缓解，痔疮体积由3.0 cm×3.0 cm缩小至1.0 cm×0.5 cm，疼痛数字评分法（NRS）评分由5分降至1分，肛肠疾病症状评分量表评分由9分降为2分，匹兹堡睡眠质量指数评分由9分降至6分，无护理相关并发症。患者产后4 d顺利出院，出院后随访1周，患者痔疮未复发加重。

结论

采用底轮灸联合输液式中药湿敷的综合中医护理方案，相较于单一护理措施，能更快速减轻痔疮水肿、缓解疼痛，且操作简便，值得临床推广。

Abstract

Objective

To summarize the Traditional Chinese Medicine （TCM） nursing experience for a parturient who developed acute inflammatory edematous external hemorrhoids following vaginal delivery.

Methods

Based on pattern （syndrome） differentiation， low-intensity rotating moxibustion combined with infusion-type Chinese herbal wet compress was adopted as the core intervention， supplemented by comprehensive measures including dietary guidance， analgesic management， emotional care， and lifestyle instruction.

Results

Following the intervention， the patient's symptoms were significantly relieved. The hemorrhoid volume decreased from 3.0 cm × 3.0 cm to 1.0 cm × 0.5 cm； the Numerical Rating Scale （NRS） pain score decreased from 5 to 1； the Anorectal Disease Symptom Scale score decreased from 9 to 2； and the Pittsburgh Sleep Quality Index （PSQI） score decreased from 9 to 6. No nursing-related complications occurred. The patient was discharged uneventfully on postpartum day 4， and a one-week telephone follow-up after discharge confirmed no recurrence or aggravation of the hemorrhoids.

Conclusion

Compared with single nursing measures， the comprehensive TCM nursing protocol integrating low-intensity rotating moxibustion with infusion-type Chinese herbal wet compress can more rapidly alleviate hemorrhoid edema and relieve pain. This approach is simple to perform and worthy of clinical promotion.
