摘要

本文报告1例十二指肠恶性肿瘤患者接受胃镜下空肠营养管置入术后的中西医结合护理经验。术前采用营养风险筛查2002（NRS-2002）、视觉模拟评分法（VAS）、焦虑自评量表（SAS）进行全面评估，同时实施心理疏导与中医情志干预；术中默契配合，确保置管成功。术后除常规管路护理与肠内营养护理外，结合中医辨证实施穴位按摩（足三里、中脘及天枢）以缓解疼痛腹胀，吴茱萸敷贴神阙穴以降逆止呕，五行音乐疗法（宫调）以安神定志。经过系统化护理，患者营养状况改善（血清白蛋白由28.6 g/L回升至32.1 g/L），疼痛评分（VAS由7分降至1分）与焦虑评分（SAS由73分降至23分）显著下降，未发生相关并发症，生活质量得到提升。

Abstract

This article reports the experience of combined traditional Chinese and Western medicine nursing for a patient with malignant tumor of the duodenum after undergoing gastroscopy-assisted jejunal nutrition tube insertion. Before the operation， a comprehensive assessment was conducted using the Nutritional Risk Screening 2002 （NRS-2002）， Visual Analogue Scale （VAS）， and Self-Rating Anxiety Scale （SAS）， and psychological counseling and traditional Chinese emotional intervention were implemented. During the operation， close cooperation was maintained to ensure the successful insertion of the tube. After the operation， in addition to routine tube care and enteral nutrition care， acupoint massage （at Zusanli， Zhongwan， and Tianqiu） was carried out based on traditional Chinese syndrome differentiation to relieve pain and abdominal distension， and Wujiuhu was applied to the Shenzhu point to suppress reflux and stop vomiting. The Five-Element Music Therapy （Gong Diao） was used to calm the mind and stabilize the spirit. Through systematic nursing， the patient's nutritional status improved （serum albumin increased from 28.6 g/L to 32.1 g/L）， the pain score （VAS decreased from 7 points to 1 point） and anxiety score （SAS decreased from 73 points to 23 points） significantly decreased， no related complications occurred， and the quality of life was enhanced.
