摘要

本文总结1例会阴部痈患者的医护一体化护理查房经验。患者因脾胃不和、气血运行失常，热盛肉腐引发“痈”症，医护团队以“扶正祛邪、标本兼治”为原则开展医护一体化护理查房，并运用查房结果指导诊疗工作。治疗上予抗感染、控制血糖、纠正贫血及电解质紊乱；护理上多措并举并充分发挥中医特色优势，予复方黄柏液冲洗联合湿敷创口，以清热利湿、修复创面，予耳穴贴压（取穴神门、皮质下、交感等）疏通经络、镇静止痛，予隔姜灸（取穴中脘、足三里及三阴交等）温通经络、健脾和胃，促进营养吸收。经治疗，患者会阴部痈完全治愈，脾胃功能改善，疾病症状缓解，康复出院。

Abstract

This article summarizes the experience of integrated medical-nursing ward rounds in the management of a patient with perineal carbuncle. The patient's condition was attributed to spleen-stomach disharmony and dysfunction of qi and blood circulation， with exuberant heat generating tissue necrosis and culminating in carbuncle formation. Guided by the therapeutic principles of "supporting healthy qi and eliminating pathogenic factors" and "addressing both the manifestation and the root cause，" the medical and nursing team conducted integrated ward rounds and applied the findings to direct clinical decision-making. The therapeutic regimen included anti-infection therapy， glycemic control， and correction of anemia and electrolyte imbalance. Nursing interventions encompassed comprehensive measures alongside distinctive traditional Chinese medicine （TCM） modalities： irrigation and wet compress with Compound Huangbai Liquid to clear heat， drain dampness， and promote wound repair； auricular acupoint sticking and pressing at Shenmen， Subcortex， and Sympathetic points to unblock meridians， tranquilize the mind， and relieve pain； and ginger-separated moxibustion at Zhongwan， Zusanli， and Sanyinjiao to warm and activate meridians， strengthen the spleen， harmonize the stomach， and facilitate nutrient absorption. Following treatment， the perineal carbuncle resolved completely， with marked improvement in spleen-stomach function， alleviation of clinical symptoms， and eventual recovery and discharge.
