摘要

目的

总结高龄脑卒中合并3期压力性损伤患者的中西医结合居家护理经验。

方法

对1例81岁男性脑卒中合并3期压力性损伤患者实施中西医结合居家护理干预，干预措施包含创面清创护理、西药换药、拔毒生肌粉外敷及耳穴压豆等。护理期间采用Braden压疮风险评估量表评估患者压力性损伤发生风险，应用创面愈合评分量表（PUSH）动态监测创面愈合情况。

结果

经38 d居家护理、累计16次创面换药干预后，患者压力性损伤创面由7.0 cm×5.0 cm×1.5 cm缩小至3.5 cm×2.0 cm×0.2 cm，PUSH评分由17分降至6分；创面坏死组织完全清除，创面全部为红色肉芽组织，大部分区域可见上皮组织生长，创面渗液明显减少，无新生坏死组织，创面呈持续愈合状态。

结论

中西医结合居家护理可有效促进高龄脑卒中合并3期压力性损伤患者的创面愈合，改善患者生活质量，在社区居家护理工作中具备良好的可行性与临床应用效果。

Abstract

Objective

To summarize the experience of integrated Chinese and Western medicine home-based nursing care for an elderly stroke patient complicated with a stage 3 pressure injury.

Methods

An 81-year-old male stroke patient with a stage 3 pressure injury received integrated Chinese and Western medicine home-based nursing interventions， including wound debridement care， Western medicine dressing change， external application of "Badu Shengji" （detoxifying and tissue-regenerating） powder， and auricular acupressure. During the care period， the Braden Pressure Ulcer Risk Assessment Scale was used to evaluate the risk of pressure injury， and the Pressure Ulcer Scale for Healing （PUSH） was applied to dynamically monitor wound healing.

Results

After 38 days of home-based nursing care with a total of 16 wound dressing changes， the pressure injury wound reduced from 7.0 cm×5.0 cm×1.5 cm to 3.5 cm×2.0 cm×0.2 cm， and the PUSH score decreased from 17 to 6. The necrotic tissue was completely removed， the wound was entirely covered with red granulation tissue， epithelial regeneration was observed in most areas， wound exudate was significantly reduced， no new necrotic tissue developed， and the wound remained in a continuous state of healing.

Conclusion

Integrated Chinese and Western medicine home-based nursing care can effectively promote wound healing in elderly stroke patients with stage 3 pressure injuries and improve their quality of life， demonstrating favorable feasibility and clinical application in community home-based nursing practice.
