摘要

本文总结1例遗传性球形红细胞增多症（HS）合并胆道术后患者行脾切除术的护理经验。患者因遗传性球形红细胞增多症长期存在溶血、黄疸及胆石症表现，先后接受胆囊切除术、胆管切开取石术及脾切除术。护理重点围绕围手术期各类护理难点制定对应干预对策，严密监测患者生命体征、引流液性状及实验室相关指标，积极预防出血、感染及血栓等术后并发症；落实精细化管道护理，保障引流管路通畅；实施个性化健康教育，提升患者疾病自我管理能力。经系统化护理干预，患者术后恢复良好，未发生严重并发症，于术后19 d康复出院。本病例提示，针对复杂遗传性球形红细胞增多症患者，开展精细化、个性化护理是保障手术成功，促进患者术后康复的关键。

Abstract

This article summarizes the nursing experience of caring for a patient with hereditary spherocytosis （HS） undergoing splenectomy following prior biliary surgery. The patient had long-standing manifestations of hemolysis， jaundice， and cholelithiasis associated with hereditary spherocytosis， and had successively undergone cholecystectomy， choledocholithotomy， and splenectomy. Nursing priorities were directed toward addressing the various perioperative challenges through tailored interventions， including close monitoring of vital signs， characteristics of drainage fluid， and relevant laboratory indicators； proactive prevention of postoperative complications such as bleeding， infection， and thrombosis； meticulous drainage tube management to ensure patency； and individualized health education to enhance the patient's capacity for self-management of the disease. Following systematic nursing interventions， the patient recovered uneventfully after the procedure without serious complications and was discharged on postoperative day 19. This case suggests that， for patients with complex hereditary spherocytosis， refined and individualized nursing care is essential to ensure surgical success and promote postoperative recovery.
