摘要

本文总结1例高龄冠心病合并三高（高血压、高脂血症、2型糖尿病）患者便秘的中西医结合护理经验。该案例基于《黄帝内经》“浊气归心”理论，采用降浊护心法护理策略，以中药保留灌肠通腑降浊、砭石疗法温通经络及耳穴疗法调畅气机为核心措施，辅以排便监测及饮食调护等西医护理手段。经干预后，患者便秘症状得到有效改善，从平素需依赖灌肠排便恢复至每日自主排便1次，排便全程未出现胸闷、心悸等并发症，为高龄冠心病合并便秘患者的中西医结合护理提供了新思路。

Abstract

This article summarizes the nursing experience of integrating traditional Chinese and Western medicine in managing constipation in an elderly patient with coronary heart disease complicated by the "three highs" （hypertension， hyperlipidemia， and type 2 diabetes mellitus）. Grounded in the theory of "turbid qi converging in the heart" （zhuoqi gui xin） from the Huangdi Neijing （The Yellow Emperor's Classic of Internal Medicine）， a nursing strategy based on the Turbidity-Reducing and Heart-Protecting Method was implemented. The core interventions comprised retention enema with Chinese herbal medicine to unblock the fu-organs and reduce turbidity， bian-stone therapy to warm and unblock the meridians， and auricular acupoint therapy to regulate qi movement， supplemented by Western nursing measures including defecation monitoring and dietary regulation. Following the intervention， the patient's constipation was effectively alleviated， with bowel movements restored from dependence on enemas to spontaneous daily defecation. No complications such as chest tightness or palpitations occurred throughout the defecation process. This case provides new insights into the integrated traditional Chinese and Western medicine nursing care for elderly patients with coronary heart disease complicated by constipation.
