摘要

本文总结1例脑干梗死合并Ⅰ型呼吸衰竭患者的中西医结合护理经验。患者入院时氧分压49.40 mm Hg，明确诊断为Ⅰ型呼吸衰竭。在常规西医治疗基础上，联合肺康复训练（腹式呼吸、有效咳嗽训练等）及中医辨证施护（耳穴压丸、穴位贴敷及针灸等）干预。干预28 d后，患者血氧饱和度由87.20%升至96.00%，洼田饮水试验由Ⅳ级改善至Ⅱ级，日常生活活动能力（ADL）评分由10分升至35分，提示中西医结合肺康复护理对脑干梗死合并呼吸衰竭患者具有积极效果。

Abstract

This paper summarizes the integrated traditional Chinese and Western medicine nursing experience for one patient with brainstem infarction complicated by type I respiratory failure. The patient had a partial pressure of oxygen of 49.40 mmHg on admission， and was definitively diagnosed with type I respiratory failure. On the basis of conventional Western medical treatment， combined interventions were delivered， including pulmonary rehabilitation training （diaphragmatic breathing， effective cough training， etc.） and traditional Chinese medicine （TCM） nursing based on syndrome differentiation （auricular acupressure， acupoint application， acupuncture and moxibustion， etc.）. After 28 days of intervention， the patient's blood oxygen saturation increased from 87.20% to 96.00%， the Kubota Water Swallow Test grade improved from Grade IV to Grade II， and the Activity of Daily Living （ADL） score rose from 10 points to 35 points， suggesting that integrated traditional Chinese and Western medicine pulmonary rehabilitation nursing exerts positive effects on patients with brainstem infarction complicated by respiratory failure.
