摘要

总结1例混合痔合并肛裂术后患者应用叙事护理联合耳穴压丸缓解疼痛的护理经验。采用多模式镇痛分级管理，轻度疼痛时实施耳穴压丸联合主题访谈式叙事护理；中重度疼痛时配合药物镇痛；辅以中药热奄包联合温和灸预防尿潴留，中药熏洗缓解术后疼痛及肛缘水肿，实施规范化创面换药管理。经精心治疗与照护，患者术后72 h首次自主排便成功，出院前疼痛及焦虑症状缓解；术后2个月随访，伤口愈合良好，肛门无狭窄、无便血。叙事护理联合耳穴压丸可从心理与生理双重路径协同干预混合痔合并肛裂术后疼痛，有效打破“疼痛—恐惧—便秘”恶性循环，促进患者术后康复。

Abstract

This paper summarizes the nursing experience of narrative nursing combined with auricular point pressing pills in alleviating pain of a patient with mixed hemorrhoids complicated with anal fissure after operation. Multimodal analgesia hierarchical management was adopted： auricular point pressing pills combined with thematic interview narrative nursing was performed for mild pain； drug analgesia was cooperated for moderate and severe pain； Chinese herbal hot compress combined with mild moxibustion was supplemented to prevent urinary retention， Chinese herbal fumigation was applied to relieve postoperative pain and anal margin edema， and standardized wound dressing change management was implemented. After careful treatment and care， the patient achieved the first spontaneous defecation successfully 72 hours after operation， and pain and anxiety symptoms were relieved before discharge； follow-up at 2 months after operation showed that the wound healed well without anal stenosis or hemafecia. Narrative nursing combined with auricular point pressing pills can synergistically intervene postoperative pain of mixed hemorrhoids complicated with anal fissure through dual pathways of psychology and physiology， effectively break the vicious circle of "pain-fear-constipation" and promote postoperative rehabilitation of patients.
