摘要

目的

探讨脐灸联合耳穴序贯技术对慢性盆腔炎湿热瘀结证患者疼痛、睡眠及焦虑的干预效果。

方法

对1例慢性盆腔炎伴痛眠障碍患者实施分阶段中医护理，初期采用常规中医外治；止痛效果欠佳后调整为以脐灸通腑祛湿为主，配合耳部按摩—耳部刮痧—耳穴贴压序贯安神助眠，联合中药灌肠、中药贴敷等综合干预，动态辨证施护。

结果

干预后患者疼痛数字评价量表（NRS）评分由6分降至1分，匹兹堡睡眠质量指数（PSQI）评分由13分降至6分，焦虑自评量表（SAS）评分由58分降至36分，夜间睡眠时间由不足5 h延长至6～7 h，患者疼痛、睡眠障碍及焦虑相关症状均得到明显改善。

结论

脐灸联合耳穴序贯技术可有效打破慢性盆腔炎“痛—眠—情”恶性循环，体现中医整体护理与辨证施护的优势，安全有效，值得临床推广。

Abstract

Objective

To explore the intervention effects of umbilical moxibustion combined with sequential auricular acupoint therapy on pain， sleep quality， and anxiety in patients with chronic pelvic inflammatory disease （CPID） presenting with the damp-heat stasis syndrome.

Methods

One patient with chronic pelvic inflammatory disease complicated by pain and sleep disorders received staged Traditional Chinese Medicine （TCM） nursing interventions. In the initial stage， conventional TCM external treatments were applied； however， with unsatisfactory analgesic effects， the regimen was adjusted to focus on umbilical moxibustion for unblocking the fu organs and eliminating dampness， supplemented by sequential auricular acupoint therapy—comprising auricular massage， auricular scraping， and auricular acupoint pressing—for calming the mind and promoting sleep. This was combined with comprehensive interventions including Traditional Chinese Medicine enema and TCM herbal application， along with dynamic syndrome differentiation-based nursing care.

Results

Following the intervention， the patient's Numerical Rating Scale （NRS） score for pain decreased from 6 to 1， the Pittsburgh Sleep Quality Index （PSQI） score decreased from 13 to 6， and the Self-Rating Anxiety Scale （SAS） score decreased from 58 to 36. Nighttime sleep duration was extended from less than 5 hours to 6–7 hours. The patient's symptoms related to pain， sleep disturbance， and anxiety were all significantly improved.

Conclusion

Umbilical moxibustion combined with sequential auricular acupoint therapy can effectively interrupt the "pain-sleep-emotion" vicious cycle in chronic pelvic inflammatory disease， demonstrating the advantages of holistic TCM nursing and syndrome differentiation-based care. This approach is safe and effective， and therefore worthy of clinical promotion.
