摘要

本报告详述1例慢性阻塞性肺疾病（COPD，稳定期GOLD 3级）合并功能性便秘（肺脾气虚兼痰浊证）患者的中西医结合护理过程。在常规治疗基础上，应用基于中医“肺与大肠相表里”理论的“砭石温熨联合呼吸引导按摩”方案进行干预。通过为期2周的个体化施护，患者便秘主症显著缓解，排便频率恢复至1次/1～2 d，腹胀消失，便秘症状评估量表总分由16分（重度）降至4分（轻度），且呼吸困难主观感受有所改善。本方案将中医外治技术与现代呼吸康复理念相结合，为COPD相关便秘的护理提供了安全、有效的非药物干预思路。

Abstract

This report details the integrated traditional Chinese and Western medicine nursing process for a patient with chronic obstructive pulmonary disease （COPD， GOLD stage 3 in stable phase） complicated by functional constipation （pattern of lung-spleen qi deficiency with phlegm-turbidity）. On the basis of conventional treatment， an intervention program of "Bian-stone hot ironing combined with breathing-guided massage，" rooted in the traditional Chinese medicine （TCM） theory of "the exterior-interior relationship between the lung and the large intestine，" was applied. Through a two-week individualized nursing regimen， the patient's primary constipation symptoms were significantly alleviated， with defecation frequency restored to once every 1-2 days， abdominal distension resolved， and the total score on the constipation symptom assessment scale decreased from 16 （severe） to 4 （mild）. Meanwhile， subjective dyspnea perception showed improvement. This program integrates TCM external treatment techniques with modern respiratory rehabilitation concepts， providing a safe and effective non-pharmacological intervention approach for the nursing care of COPD-related constipation.
