摘要

慢性肾衰竭（CRF）患者常并发下肢水肿，严重影响其生活质量与健康状况，因此，有效的治疗与护理至关重要。本案例采用中药塌渍联合超声药物透入治疗，旨在改善慢性肾衰患者的下肢水肿症状。中药塌渍选用具有利水消肿功效的中药方剂，外敷于患者下肢，发挥局部治疗作用；超声药物透入则利用超声波促进药物渗透，增强治疗效果。治疗过程中，护理要点涵盖治疗前对患者身体状况、皮肤情况及心理状态的全面评估，治疗中对药物温度、仪器操作及患者反应的密切观察，以及治疗后皮肤护理与病情监测。经该方案治疗护理后，患者下肢水肿显著改善，舒适度提升，肾功能指标也有所好转。

Abstract

Lower limb edema is a common complication in patients with chronic renal failure （CRF）， which seriously compromises both quality of life and clinical outcomes. Therefore， effective treatment and nursing care are of critical importance. In this case report， traditional Chinese medicine （TCM） compress combined with ultrasound-mediated drug penetration therapy was employed to alleviate lower limb edema in a patient with CRF. The TCM compress was formulated with Chinese herbal medicines possessing diuretic and anti-edema properties， and was topically applied to the affected lower limbs to exert localized therapeutic effects. Ultrasound-mediated drug penetration was utilized to facilitate transdermal drug absorption and enhance therapeutic efficacy. Throughout the treatment course， nursing interventions encompassed the following key aspects： a comprehensive pre-treatment assessment of the patient's general physical condition， skin integrity， and psychological status； close intra-treatment monitoring of herbal decoction temperature， instrument operation， and patient response； and post-treatment skin care along with ongoing evaluation of the patient's condition. Following the implementation of this combined therapeutic and nursing regimen， the patient demonstrated marked improvement in lower limb edema， enhanced comfort， and favorable changes in renal function indicators.
