本文总结1例36

周早产儿胸腔积液（乳糜胸）的护理经验，在护理程序中聚焦患儿核心护理问题，在精细化护理理念的指导下给予引流管规范化维护、个体化俯卧位通气、阶梯式精准喂养、全流程感染防控、动态疼痛评估与舒适安抚、家属心理疏导及照护赋能等护理干预。最终患儿生命体征平稳，成功撤离呼吸支持与静脉营养，喂养耐受良好，家属焦虑缓解，顺利康复。本案中个体化精细化管理联合家庭参与式护理是患儿治疗成功的关键，可为今后的护理工作提供参考。

Abstract

This article summarizes the nursing experience of one preterm infant born at 36⁺² weeks of gestation diagnosed with pleural effusion （chylothorax）. Focusing on the core nursing problems identified through the nursing process， a series of nursing interventions were implemented under the guidance of the refined nursing concept， including standardized drainage tube maintenance， individualized prone positioning ventilation， stepped precise enteral feeding， whole-process infection prevention and control， dynamic pain assessment with comfort care， as well as psychological counseling and care empowerment for the family. Ultimately， the infant's vital signs remained stable， with successful weaning from both respiratory support and parenteral nutrition. Feeding tolerance was favorable， family anxiety was alleviated， and the patient recovered smoothly. In this case， individualized refined management combined with family-participatory nursing care proved critical to the success of treatment， which may serve as a reference for future clinical nursing practice.
