摘要
失禁相关性皮炎是指由于暴露于尿液或粪便所造成的皮肤损伤。本文总结了1例基于循证的老年患者失禁性皮炎护理经验。主要以《成人失禁相关性皮炎护理专家共识》（2020）为依据，构建护理计划，动态评估患者失禁性皮炎风险分级、严重程度，针对评估结果结合证据制定结构化护理方案。从风险因素去除、中西医结合的皮肤清洗与保护方式的应用等完成护理干预，有效改善了患者失禁性皮炎。循证个性化护理实践有助于推动证据转化，改善患者的预后，能够进一步提升临床护理质量。
Abstract
Incontinence-associated dermatitis refers to skin damage caused by exposure to urine or feces. The article summarized the nursing experience of an elderly patient with incontinence dermatitis received evidence-based nursing. Medical and nursing staff had built a nursing plan and assessed dynamically incontinent dermatitis risk classification and severity of patient according to the adult incontinent dermatitis correlation nursing expert consensus （2020）， and formulated a structured nursing plan based on evidence. The nursing intervention was completed from the removal of risk factors， the application of skin cleaning and protection methods combined with traditional Chinese and western medicine， which effectively relieved symptoms of the incontinence-associated dermatitis. Evidence-based personalized nursing practice was helpful to promote evidence transformation， improve the prognosis of the patient， and further improve the quality of clinical nursing.
