摘要

先天性结核病病情进展迅速，死亡率高，如果不及早发现，预后很差。由于非特异性临床表现、误诊和妊娠期未确诊的母亲结核病，诊断常常被延误，尤其是早产儿。本文总结了一例超低出生体重双胎之大合并先天性结核病在新生儿重症监护室的护理经验。护理要点包括：消毒隔离和防护措施、气道护理、静脉通路护理、体温皮肤护理、用药护理、家长心理支持。双胎之小入院后病情进一步恶化，家属放弃抢救；双胎之大，病情加重，转院治疗。

Abstract

Congenital tuberculosis progresses rapidly with high mortality. If it is not found early， the prognosis is poor. Because of non-specific clinical manifestations， misdiagnosis and mother's tuberculosis not diagnosed during pregnancy， diagnosis is often delayed， especially for premature infants. This article summarized the nursing experience of a case of twin with extremely low birth weight and congenital tuberculosis in the NICU. The key points of nursing care included ：disinfection， isolation and protective measures， airway care， venous access care， temperature and skin care， medication care， and parental psychological support. The condition of the twin became worse after admission， and the family members gave up rescue； The twin is big， the condition is aggravated， and the patient is transferred to hospital for treatment.
