摘要

本文总结1例应用五位一体护理模式照护的克罗恩病患者护理经验。以药物治疗、特色中医护理疗法、康复指导、健康教育、情志护理为五位一体的全程护理模式，多元化为患者提供临床护理。患者出院后，开展延续性护理，通过微信、电话等方式持续追踪。结果显示，五位一体护理模式护理效果明显，患者认可度高，值得临床推广。

Abstract

This article summarizes the nursing experience of a Crohn's disease patient who applied the five in one nursing model. The comprehensive nursing model that integrates medication therapy， characteristic TCM nursing therapy， rehabilitation guidance， health education， and emotional care， providing diversified clinical care was applied. After the patient is discharged， continuity care is carried out and continuous tracking was conducted through WeChat， phone， and other means. The results show that the five in one nursing model has significant nursing effects and high patient recognition， which is worthy of clinical promotion.
