摘要

回顾2024年11月15日收治的1例胆总管下段癌患者，该患者在全身麻醉下接受胰十二指肠切除术。术后第10天患者并发胃轻瘫，经中西医联合治疗得到快速康复。本文旨在探讨中西医结合治疗胃轻瘫快速康复的护理经验。本例患者为老年男性，因“胆囊炎，肝功能不全”入院治疗，经全腹+盆腔多期强化CT检查，发现胆总管远端及胰头区占位性病变，初步诊断为胆总管下段癌。术前各项准备工作完成后，患者在全身麻醉下接受“胰十二指肠切除术”。术后第10天夜间，患者出现腹痛、腹胀、恶心、呕吐及呃逆症状，呕吐物量达600 mL，为胃内食物残渣。CT影像学检查揭示胃部扩张及内容物滞留，从而确诊为术后胃轻瘫。随即对患者实施禁食及留置胃管胃肠减压治疗，减压量为290 mL。次日晨，邀请中医专家进行会诊，中医专家采用针灸及芒硝腹部外敷进行辅助治疗。至胃瘫后第3日，胃肠减压量减少至50 mL，患者腹痛症状有所缓解，但仍有腹胀及呃逆主诉。随后继续结合胃动力药物治疗、中医穴位针灸，芒硝外敷、血糖控制、营养管理、活动锻炼指导及精神压力缓解等综合措施，至胃瘫后第4天胃肠减压量进一步减少至5 mL，患者腹痛、腹胀、恶心及呕吐症状均得到显著缓解。拔除胃管后，患者经口进食米汤无不适反应，逐步由流质饮食过渡至半流质饮食。经过7 d综合治疗，患者胃瘫症状得到缓解。中医针灸与芒硝外敷，结合胃动力药物、血糖控制、营养加强、活动锻炼和心理护理，对术后胃瘫具有显著的治疗效果，能够促进术后胃肠道功能恢复，减轻患者术后腹部疼痛，提高生活质量，缩短病程，值得在临床实践中广泛推广。

Abstract

This paper reviews a case of a patient with cholangiocarcinoma at the lower segment of the common bile duct who was admitted on November 15， 2024， under general anesthesia and underwent pancreaticoduodenectomy. On the 10th day after the operation， the patient developed gastroparesis and achieved rapid recovery through combined traditional Chinese and Western medicine treatment. This paper aims to explore the nursing experience of rapid recovery of gastroparesis patients treated with integrated traditional Chinese and Western medicine. The patient was an elderly male who was admitted for "cholecystitis， liver dysfunction". After comprehensive abdominal and pelvic multi-phase enhanced CT examination， a mass lesion was found in the distal common bile duct and pancreatic head area， and the preliminary diagnosis was cholangiocarcinoma at the lower segment of the common bile duct. After completing all preoperative preparations， the patient underwent "pancreaticoduodenectomy" under general anesthesia. On the night of the 10th day after the operation， the patient presented with abdominal pain， abdominal distension， nausea， vomiting， and hiccups， with a vomiting volume of 600 mL， which was gastric food residue. CT imaging revealed gastric dilation and retention of contents， thus confirming postoperative gastroparesis. Immediately， the patient was given fasting and gastric tube intubation for gastrointestinal decompression treatment， with a decompression volume of 290 mL. The next morning， a consultation with an expert in traditional Chinese medicine was invited， and acupuncture and abdominal external application of mirabilite were used for auxiliary treatment. On the 3rd day after gastroparesis， the decompression volume decreased to 50 mL， and the abdominal pain symptoms of the patient were alleviated， but there were still complaints of abdominal distension and hiccups. Subsequently， combined measures such as gastric motility drug treatment， acupuncture at specific acupoints， abdominal external application of mirabilite， blood sugar control， nutritional management， activity exercise guidance， and stress relief were continued. On the 4th day after gastroparesis， the decompression volume further decreased to 5 mL， and the abdominal pain， abdominal distension， nausea， and vomiting symptoms of the patient were significantly relieved. After the gastric tube was removed， the patient was able to consume rice water orally without discomfort， and gradually transitioned from liquid diet to semi-liquid diet. After 7 days of comprehensive treatment， the patient's gastroparesis symptoms were relieved. The combination of traditional Chinese acupuncture and external application of mirabilite， combined with gastric motility drugs， blood sugar control， nutritional enhancement， activity exercise guidance， and psychological care， has a significant therapeutic effect on postoperative gastroparesis， promoting the recovery of postoperative gastrointestinal function， reducing abdominal pain after surgery， improving quality of life， shortening the course of the disease， and is worthy of wide promotion in clinical practice.
