摘要
丹毒是一种由溶血性链球菌感染引起的急性炎症性疾病，主要侵犯皮肤及其网状淋巴管系统，表现为局部红肿、疼痛及全身发热。肥胖及慢性病的高发使护理管理更具挑战性，因静脉回流障碍、皮肤屏障受损及活动受限，更易出现并发症。本案例通过分析1例大体重下肢丹毒合并高血压、冠心病患者的护理过程，探讨特色护理和系统化护理干预的应用价值。护理过程中采用梯度式抬高患肢和交替翻身，渐进式活动管理确保活动恢复的安全性，系统化护理干预有效控制感染，改善患者依从性。同时帮助患者优化生活方式，在体重管理、饮食控制及戒烟等方面取得积极成效。案例表明特色护理结合系统化护理干预，能有效提升合并慢性病丹毒患者的康复效果，为临床护理提供重要参考。
Abstract
Erysipelas is an acute inflammatory disease caused by infection with hemolytic streptococcus， mainly affecting the skin and its reticular lymphatic system， presenting with local redness， swelling， pain and systemic fever. The high incidence of obesity and chronic diseases makes nursing management more challenging. Due to venous return obstruction， damaged skin barrier and limited activity， complications are more likely to occur. This case study analyzes the nursing process of a patient with lower limb erysipelas combined with hypertension and coronary heart disease of large body weight， and explores the application value of characteristic nursing and systematic nursing intervention. During the nursing process， gradient elevation of the affected limb and alternate turning were adopted， and progressive activity management ensured the safety of activity recovery. Systematic nursing intervention effectively controlled the infection and improved patient compliance. At the same time， it helped the patient optimize their lifestyle and achieved positive results in weight management， diet control and smoking cessation. The case indicates that the combination of characteristic nursing and systematic nursing intervention can effectively improve the rehabilitation effect of patients with erysipelas complicated with chronic diseases， providing an important reference for clinical nursing.
