摘要
本报告描述2例酒精性肝硬化患者在酒精戒断期间使用哌拉西林钠他唑巴坦钠（邦达）前皮试结果为阴性，但是在输注过程中发生过敏反应，最终通过积极救治抢救成功的案例。根据患者发生过敏反应的时间、临床表现、结合患者病情、进行过敏原因分析并经验总结。2例患者均为酒精戒断状态，输注哌拉西林钠他唑巴坦钠前均口服镇静药物，根据青霉素皮肤试验专家操作共识及相关指南可知默写药物可对其皮试结果有抑制作用，造成皮试结果不准确。本报告旨在提示医务人员关注药物对β内酰胺类药物皮肤试验结果的影响；对于皮肤过敏试验结果为阴性的患者，在药物输注过程中仍需保持警惕，确保用药安全；同时强调护理人员工作中对患者用药宣教及掌握过敏性休克急救知识及的重要性。
Abstract
This report describes two cases of patients with alcoholic cirrhosis who experienced allergic reactions during the infusion of piperacillin sodium and tazobactam sodium （Bonda） despite negative skin tests before infusion. The patients were in an alcohol withdrawal state and took sedative drugs orally before the infusion. The allergic reactions were analyzed based on the time of occurrence， clinical manifestations， and the patients' conditions. The causes of the allergic reactions were analyzed and experiences were summarized. Both patients were in an alcohol withdrawal state and took sedative drugs orally before the infusion. According to the expert consensus on skin tests for penicillin and relevant guidelines， the written drugs can inhibit the skin test results， resulting in inaccurate results. This report aims to alert medical staff to pay attention to the impact of drugs on the skin test results of β-lactam antibiotics； for patients with negative skin test results for allergic reactions， vigilance is still necessary during drug infusion to ensure medication safety； at the same time， it emphasizes the importance of nursing staff's education on medication use for patients and their mastery of knowledge and skills for emergency treatment of anaphylactic shock.
