摘要

本文总结1例70岁高龄盆腔恶性肿瘤患者行根治性切除术中出现20000 mL大出血的急救护理经验。该病例肿瘤体积达23 cm×10 cm×9.6 cm，侵犯膀胱、输尿管及骶前血管，术中突发左侧睾丸动脉破裂，合并心功能不全、高同型半胱氨酸血症等高危因素，手术风险极高。护理团队通过多学科协作模式，实施分层预警管理，即术前采用Caprini量表评估VTE风险（17分），制定个性化预案；术中启动“三级输血响应机制”，联合介入栓塞止血，同步落实体温保护、压力性损伤预防及循环支持；术后强化血栓防控与心理干预。患者术后17 d康复出院，未发生并发症。本案例提示针对高龄复杂肿瘤患者，建立以“快速输血—精准止血—多维度防护”为核心的多学科协作护理路径，可显著提升急救效率，降低并发症风险。

Abstract

This paper summarizes the emergency nursing experience of a 70-year-old patient with a pelvic malignant tumor who underwent radical resection and experienced a massive intraoperative hemorrhage of 20，000 mL. The tumor measured 23 cm × 10 cm × 9.6 cm and invaded the bladder， ureter， and presacral vessels. Intraoperatively， a sudden rupture of the left testicular artery occurred， along with high-risk factors such as heart failure and hyperhomocysteinemia， making the surgical risk extremely high. The nursing team adopted a multidisciplinary collaboration model and implemented a stratified early warning management system. Preoperatively， the Caprini scale was used to assess the risk of venous thromboembolism （VTE）（17 points）， and a personalized plan was developed. Intraoperatively， a "three-level blood transfusion response mechanism" was initiated， combined with interventional embolization for hemostasis， while simultaneously implementing temperature protection， pressure injury prevention， and circulatory support. Postoperatively， thrombosis prevention and psychological intervention were strengthened. The patient was discharged 17 days after the operation without any complications. This case suggests that for elderly patients with complex tumors， establishing a multidisciplinary collaboration nursing pathway centered on "rapid blood transfusion - precise hemostasis - multi-dimensional protection" can significantly improve emergency response efficiency and reduce the risk of complications.
