摘要
本案例报告1例左足Wagner3级坏疽患者，在血管介入重建血运（股动脉支架+球囊扩张）基础上，结合中医鲸吞清创、金黄散箍围与复方黄柏液冷敷，同步强化动态血糖管理（胰岛素泵+CGMS），最终实现创面愈合率达70%、疼痛显著缓解。本病例特异性在于多重并发症共存（坏疽、神经病变、血管病变、脑梗死），病情复杂，截肢风险高。本案例凸显内外同治、中西医结合护理在延缓病情进展、降低截肢率中的价值，强调个性化干预与跨团队协作的重要性，为复杂糖尿病足管理提供实践参考。
Abstract
This case report describes a patient with left foot Wagner Grade 3 gangrene who underwent vascular interventional revascularization （femoral artery stenting with balloon angioplasty） combined with Traditional Chinese Medicine （TCM） interventions， including whale-swallowing debridement， Jinhuang San （Golden Yellow Powder） encircling therapy， and compound Huangbai liquid cold compress. Dynamic glucose management was intensified using an insulin pump and continuous glucose monitoring system （CGMS）. The treatment achieved a 70% wound healing rate and significant pain relief. The uniqueness of this case lies in the coexistence of multiple complications （gangrene， neuropathy， vascular disease， and cerebral infarction）， complex clinical presentation， and high amputation risk. This case highlights the clinical value of integrated internal-external therapies and Chinese-Western collaborative nursing in delaying disease progression， reducing amputation rates， and emphasizing personalized interventions and multidisciplinary teamwork， providing practical insights for managing complex diabetic foot conditions.
