摘要

本文概述1例基于老年综合评估（CGA）的高龄脑卒中合并衰弱患者的中西医结合护理经验。根据CGA评估结果，建立多学科诊疗团队，提出护理诊断，并运用中西医结合干预方式，对患者进行个性化护理，旨在改善患者的衰弱状态，提高生活质量。

Abstract

This article outlines the nursing experience of integrated Sino-Western nursing for a geriatric stroke patient coexisting with frailty based on Comprehensive Geriatric Assessment （CGA）. Guided by CGA results， a multidisciplinary team was established to formulate nursing diagnoses. Personalized interventions integrating traditional Chinese and Western nursing approaches were implemented to ameliorate the patient’s frailty status and enhance quality of life.
