摘要
本文总结1例复发性粘连性肠梗阻（脾虚气滞证）患者的中西医结合护理实践。患者因“腹痛伴恶心呕吐1 d”入院，腹部CT示低位小肠梗阻，既往有宫颈癌放疗及多次肠梗阻病史。西医予胃肠减压、抗感染及补液治疗，中医以健脾行气通腑为法，护理方案整合中药灌肠、中药膏摩、五行音乐疗法及个性化饮食调护。结果显示，患者首次排气时间缩短至48 h，腹痛程度评分（VAS评分）由6分降至2分，住院周期为11 d（较既往缩短3 d），随访1个月无复发。本案例证实中西医协同护理可加速肠功能恢复、减少并发症，并为复发性肠梗阻的个体化护理提供参考路径，凸显中医“整体观”在急症管理中的独特价值。
Abstract
This article summarizes the practice of combined traditional Chinese and Western medicine nursing for a patient with recurrent adhesive intestinal obstruction （spleen deficiency and qi stagnation syndrome）. The patient was admitted due to "abdominal pain accompanied by nausea and vomiting for 1 day". Abdominal CT showed low-position small intestinal obstruction. The patient had a history of cervical cancer radiotherapy and multiple episodes of intestinal obstruction. The Western medicine treatment included gastrointestinal decompression， anti-infection， and fluid replacement. The traditional Chinese medicine treatment focused on strengthening the spleen and promoting qi circulation and eliminating the bowels. The nursing plan integrated Chinese herbal enema， Chinese herbal massage， five-element music therapy， and individualized dietary care. The results showed that the patient's first defecation time was shortened to 48 hours， the abdominal pain score （VAS score） decreased from 6 points to 2 points， the hospitalization period was 11 days （3 days shorter than before）， and there was no recurrence during the 1-month follow-up. This case confirmed that the collaborative nursing of traditional Chinese and Western medicine can accelerate intestinal function recovery， reduce complications， and provide a reference path for individualized nursing of recurrent intestinal obstruction， highlighting the unique value of the "holistic view" of traditional Chinese medicine in emergency management.
