摘要
本例报告1例急性淋巴细胞白血病（ALL）患者在接受经外周静脉置入中心静脉导管（PICC）置管后并发相关深静脉血栓的中西医结合护理经验。患者于PICC导管置入后出现右上肢肿胀疼痛，血管超声确诊为右侧锁骨下静脉中远段、腋静脉及肱静脉上段血栓形成，伴严重低纤维蛋白原血症。西医护理以低分子肝素抗凝、抬高患肢及外涂自备喜辽妥治疗为主；中医护理主予金黄散冷湿敷。护理后患肢肿胀消退，未发生肺栓塞及出血等严重并发症。提示对于血液肿瘤化疗患者，需高度重视PICC导管相关血栓的防治，中西医结合护理模式在改善患者症状方面具有独特优势。
Abstract
This report summarizes the integrated Chinese and Western medicine nursing experience in managing a case of deep vein thrombosis （DVT） secondary to peripherally inserted central catheter （PICC） placement in a patient with acute lymphoblastic leukemia （ALL）. The patient developed right upper limb swelling and pain post-PICC insertion， with ultrasound confirming thrombosis in the mid-distal right subclavian vein， axillary vein， and proximal brachial vein， accompanied by severe hypofibrinogenemia. Western nursing interventions included low-molecular-weight heparin anticoagulation， limb elevation， and topical application of Hirudoid cream， while traditional Chinese medicine （TCM） nursing employed cold compress with Jinhuang San （Golden Yellow Powder）. Post-intervention， limb edema resolved without complications such as pulmonary embolism or bleeding. This case highlights the necessity of vigilant thrombosis prevention in hematologic malignancy patients undergoing chemotherapy and demonstrates the synergistic benefits of integrated Chinese-Western nursing in symptom management.
