摘要

本文总结1例多发性脑梗死并C4-C6脊髓损伤患者实施清洁间歇性导尿的护理经验。入院后完善评估，制定“每4 h实施1次间歇导尿＋膀胱功能训练”方案，并配合体位减压、感染防控、康复运动及心理疏导。治疗2周后，单次残余尿量降至≤80 mL，未见尿路感染，Barthel指数由15分增至30分，SAS由76.25分降至45分，排尿安全、生活功能及情绪均明显改善。

Abstract

This article summarizes the nursing experience of implementing clean intermittent catheterization （CIC） in a patient with multiple cerebral infarctions and C4–C6 spinal cord injury. Upon admission， a comprehensive assessment was conducted， and a protocol of "intermittent catheterization every 4 hours combined with bladder function training" was established， supplemented by postural decompression， infection prevention， rehabilitation exercises， and psychological support. After two weeks of treatment， the single residual urine volume decreased to ≤80 mL， with no urinary tract infections observed. The Barthel Index improved from 15 to 30 points， while the SAS score decreased from 76.25 to 45 points， indicating significant enhancements in voiding safety， daily living function， and emotional well-being.
