摘要

本文总结应用中西医结合护理成功救治1例87岁急性肾衰竭合并糖尿病性酮症酸中毒患者的实践与经验。西医护理包括监测生命体征、动脉置管，评估血透管通畅性并以肝素盐水冲封管，行床旁血滤（CVVH模式，枸橼酸抗凝），动态监测血气电解质，予无创呼吸机、抗感染、补液纠酸、降钾、胰岛素泵控糖及鼻饲肠内营养。中医辨证为泄泻病脾胃虚证，予患者中药热奄包治疗以温脾散寒，膻中穴贴敷以宽胸理气，耳穴压丸治疗以调节脏腑，配合艾灸足三里健脾。护理中动态观察患者血滤管路、酸碱及血糖，中西医协同治疗以促进病情稳定。

Abstract

This article summarizes the practice and experience of applying integrated traditional Chinese and Western medicine （TCM-WM） nursing to successfully treat an 87-year-old patient with acute renal failure （ARF） complicated by diabetic ketoacidosis （DKA）. Western medical interventions included monitoring vital signs， arterial catheter placement， assessing hemodialysis catheter patency and maintaining it with heparinized saline flushes and locks， bedside continuous venovenous hemofiltration （CVVH mode， citrate anticoagulation）， dynamic monitoring of blood gas and electrolytes， non-invasive ventilator support， anti-infective therapy， fluid resuscitation and acidosis correction， potassium reduction， insulin pump for glycemic control， and nasogastric enteral nutrition. Traditional Chinese medicine interventions were tailored to the patient’s syndrome differentiation of "diarrhea disorder due to spleen-stomach deficiency syndrome." These included applying Chinese herbal hot compress packs to warm the spleen and dispelling cold， acupoint application at Danzhong （CV17） to regulate chest qi， auricular acupressure with magnetic beads to regulate visceral functions， and moxibustion at Zusanli （ST36） to strengthen the spleen. Throughout the care process， dynamic monitoring of hemofiltration circuit status， acid-base balance， and blood glucose was emphasized， with integrated TCM-Western interventions synergistically promoting clinical stabilization.
