摘要
本文总结1例腹膜后纤维化合并输尿管狭窄患者的失眠护理实践，重点探讨艾灸、穴位贴敷及中药热奄包等中医外治技术的干预效果。基于八纲辨证，患者属脾肾阳虚兼湿瘀内阻证，护理中采用辨证施护原则：①通过艾灸温补脾肾阳气；②以祛湿化瘀类中药穴位贴敷以疏通经络；③配合中药热奄包局部热敷缓解腰痛。同时结合情志调摄与起居饮食指导，最终患者失眠症状显著改善，腰痛程度减轻，焦虑情绪缓解，生活质量得到有效提升。
Abstract
This report summarizes the nursing management of insomnia in a patient with retroperitoneal fibrosis complicated by ureteral stricture， focusing on the therapeutic effects of external TCM techniques including moxibustion， acupoint application， and herbal hot compress. Based on the Eight-Principle Pattern Differentiation， the patient was diagnosed with spleen-kidney yang deficiency complicated by internal dampness-blood stasis. The nursing protocol followed syndrome-specific principles： （1） Moxibustion to warm and tonify spleen-kidney yang； （2） Dampness-resolving and stasis-dissolving herbal acupoint application to unblock meridians； （3） Local herbal hot compress to alleviate lumbar pain. Integrated with emotional regulation and lifestyle-dietary guidance， these interventions significantly improved insomnia symptoms， reduced lumbar pain intensity， relieved anxiety， and enhanced the patient's quality of life.
