摘要

本文总结1例具有过敏性紫癜病史的异位妊娠患者手术过程中输注异体血和自体血后发生急性溶血反应的护理体会。核心护理措施包括实施“快速识别—立即停输—多学科协作”的应急流程，发现暗红色尿立即停输自体血、启动多学科会诊；术中严格体温保护，维持鼻咽温36.0～37.0℃，使用加温输液仪；术后即刻启动呋塞米利尿及碳酸氢钠碱化尿液方案保护肾功能，术后9 h尿色从酱油色转为淡红，12 h恢复正常。通过分析该案例，旨在提升医务人员特别是对过敏体质患者输血高风险管理的意识，以及对于输血后溶血反应的快速识别、规范处置、精细护理及有效预防能力，提高输血效果，确保用血安全。

Abstract

This article summarizes the nursing management of acute hemolytic reaction occurring after allogeneic and autologous blood transfusion during surgery in an ectopic pregnancy patient with a history of allergic purpura. Key interventions included： Implementing an emergency protocol of "rapid identification–immediate cessation–multidisciplinary collaboration"， where dark red urine triggered immediate transfusion stoppage and multidisciplinary consultation.Intraoperative strict temperature maintenance （nasopharyngeal temperature 36.0–37.0°C） using a fluid warmer.Postoperative initiation of furosemide diuresis and sodium bicarbonate urine alkalinization to protect renal function， with urine color transitioning from cola-colored to light red within 9 hours and normalizing by 12 hours.This case highlights the importance of heightened vigilance for high-risk transfusion management in allergic patients， emphasizing rapid recognition， standardized treatment， meticulous nursing， and preventive strategies to ensure transfusion safety.
