摘要

本研究报告1例64岁颈痈（热毒壅结证）合并糖尿病患者的护理过程。患者后颈部红肿疼痛伴脓性渗出，面积约10 cm×10 cm，中医辨证为“营气不从，逆于肉理”，西医诊断为颈部脓肿合并2型糖尿病。采用中药塌渍法（苦参、黄柏等药物醋调外敷）联合抗感染、血糖控制等治疗，结合中医辨证护理评估与干预。本病例结合《黄帝内经》痈肿病机理论及现代护理实践，探讨中药塌渍法在复杂颈痈中的应用价值，以期为同类病例提供护理参考。

Abstract

This study reports the nursing process of a 64-year-old patient with cervical abscess （heat-toxin accumulation syndrome） complicated by diabetes mellitus. The patient presented with a 10 cm × 10 cm erythematous， painful swelling and purulent discharge on the posterior neck， diagnosed in Traditional Chinese Medicine （TCM） as "disordered nutrient qi rebelling into the muscles" and in Western medicine as cervical abscess with type 2 diabetes. Herbal compress therapy （external application of Sophora flavescens， Phellodendron amurense， etc.*， mixed with vinegar） was combined with anti-infection treatment and glycemic control， alongside TCM syndrome-differentiated nursing assessment and interventions. Integrating the pathological theory of abscess from Huangdi Neijing with modern nursing practice， this case explores the clinical value of herbal compress therapy in complex cervical abscess management， providing a reference for similar cases.
