摘要
本文总结1例缺血性中风静脉溶栓后头晕头痛患者的中医护理实践。基于中医辨证（气虚血瘀证），在常规缺血性中风护理的基础上，重点应用穴位推拿特色技术进行干预。结果显示，该干预措施有效缓解患者头晕头痛症状，头晕评估量表VSS（Vertigo Symptom Scale）评分由37分降至31分，匹兹堡睡眠质量指数（Pittsburgh Sleep Quality Index， PSQI）评分由16分降至10分，肌力由3级提升至5级，改善不良情绪，患者预后良好。本案例提示穴位推拿可作为改善缺血性中风静脉溶栓后头晕头痛症状的有效中医护理手段。
Abstract
This article summarizes the Traditional Chinese Medicine （TCM） nursing practice for a patient with dizziness and headache following intravenous thrombolysis for ischemic stroke. Based on TCM syndrome differentiation （Qi deficiency and blood stasis syndrome）， the intervention emphasized acupoint massage as a specialized technique， supplemented by conventional ischemic stroke care. Results demonstrated significant symptom relief： the Vertigo Symptom Scale （VSS） score decreased from 37 to 31， the Pittsburgh Sleep Quality Index （PSQI） score improved from 16 to 10， muscle strength increased from grade 3 to grade 5， and negative emotions were alleviated， leading to a favorable prognosis. This case suggests that acupoint massage may serve as an effective TCM nursing intervention for managing dizziness and headache post-thrombolysis in ischemic stroke patients.
