摘要
本文旨在总结中药膏摩联合中药热奄包在干预1例脑梗死后恢复期合并肩手综合征患者疼痛方面的护理体会。护理团队对患者的躯体活动、疼痛、自理能力、睡眠及心理状态进行系统评估。在遵医嘱执行常规康复与药物治疗的基础上，重点实施以问题为导向的疼痛与肿胀管理、肢体功能促进、睡眠形态干预及情志护理。依据患者“气虚血瘀”的中医证型，采用中药膏摩疗法以活血通络，并联合中药热奄包治疗以温经散寒、行气止痛。通过为期1周的综合护理，患者疼痛得到有效控制，患肢肿胀亦有改善趋势，为后续康复创造了良好条件。
Abstract
This report summarizes the nursing experience of integrating herbal ointment massage with medicinal hot compress to alleviate pain in a patient with post-stroke shoulder-hand syndrome during the recovery phase. The nursing team conducted systematic assessments of the patient’s physical mobility， pain intensity， self-care capacity， sleep quality， and psychological status. Alongside conventional rehabilitation and pharmacotherapy， targeted interventions were implemented， including problem-oriented pain and edema management， limb function promotion， sleep pattern adjustment， and emotional support. Based on the patient’s TCM syndrome differentiation （Qi deficiency and blood stasis）， herbal ointment massage was applied to activate blood circulation and unblock collaterals， while medicinal hot compress was used to warm meridians， dispel cold， and relieve pain. After one week of integrated care， the patient’s pain was effectively controlled， with a noticeable reduction in limb swelling， thereby facilitating subsequent rehabilitation.
