摘要
本文总结自制倒流烟药香灸配合穴位贴敷在1例乳腺癌患者延迟性CINV中的护理体会。患者为乳腺癌患者，于化疗第2天恶心呕吐严重，周身乏力，严重影响日常进食及生活质量。基于护理评估和中医四诊辨证，除常规护理外，每日予患者自制倒流烟药香灸及穴位贴敷的中医护理干预。本案例中自制倒流烟药香灸所用药香是采用和胃止呕的芳香中药超微粉碎后按比例加入适量艾绒，制成倒流烟塔香，将塔香放置于神阙、中脘穴点燃施灸；穴位贴敷是用健脾和胃、理气止呕的药方打成药粉，用姜汁调成膏状贴敷于双侧内关穴、足三里穴。经干预，有效减轻患者延迟性CINV症状，改善其食欲，提升患者生活质量。
Abstract
This article summarizes the nursing experience of self-made reverse-smoke medicinal moxa combined with acupoint application in treating delayed chemotherapy-induced nausea and vomiting （CINV） in a breast cancer patient. The patient， diagnosed with breast cancer， experienced severe nausea， vomiting， and generalized fatigue on the second day of chemotherapy， significantly impairing daily food intake and quality of life. Based on nursing assessment and TCM syndrome differentiation （four diagnostic methods）， in addition to routine care， the patient received daily interventions of self-made reverse-smoke medicinal moxa and acupoint application. The medicinal moxa was prepared by ultrafine grinding of aromatic Chinese herbs with antiemetic properties， blended with appropriate proportions of mugwort floss， and formed into reverse-smoke cone incense. The cones were ignited and applied at acupoints Shenque and Zhongwan. For acupoint application， a powder of herbal formula （strengthening the spleen， harmonizing the stomach， and regulating qi to relieve vomiting） was mixed with ginger juice to form a paste and applied bilaterally at Neiguan and Zusanli. The interventions effectively alleviated delayed CINV symptoms， improved appetite， and enhanced the patient’s quality of life.
