摘要
本文总结1例既往非ST段抬高型心肌梗死（NSTEMI）经皮冠状动脉介入治疗（PCI）术后患者，因持续胸痛4 h、意识障碍3 h急诊入院后突发心室颤动，经200J双向波电除颤3次、持续高质量胸外按压、胺碘酮联合肾上腺素静脉给药等抢救措施后恢复窦性心律。急诊冠状动脉造影拟行二次PCI术，但术中见胃液呈棕褐色（提示上消化道出血可能）且血压持续偏低，遂暂缓支架植入，改为替罗非班（12.5 mg/h）静脉抗栓治疗，并行气管插管容量控制机械通气（V-CMV），采用咪达唑仑+丙泊酚+舒芬太尼三联镇静镇痛方案，联合去甲肾上腺素及多巴胺维持血压。现将本例心肌梗死合并心脏骤停患者的成功抢救护理经验总结如下。
Abstract
This article summarizes the successful rescue and nursing experience of a patient with a history of non-ST-segment elevation myocardial infarction （NSTEMI） post-percutaneous coronary intervention （PCI）， who was admitted to the emergency department due to persistent chest pain for 4 hours and impaired consciousness for 3 hours， followed by sudden ventricular fibrillation. Sinus rhythm was restored after three 200J biphasic defibrillations， sustained high-quality chest compressions， and intravenous administration of amiodarone combined with epinephrine. Emergency coronary angiography was planned for secondary PCI， but the procedure was temporarily suspended due to brown gastric fluid （suggesting possible upper gastrointestinal bleeding） and persistent hypotension. Instead， tirofiban （12.5 mg/h） was administered intravenously for antithrombotic therapy， along with endotracheal intubation and volume-controlled mechanical ventilation （V-CMV）. A triple sedative-analgesic regimen （midazolam， propofol， and sufentanil） was implemented， supplemented by norepinephrine and dopamine for blood pressure support. The nursing strategies for this case of myocardial infarction complicated by cardiac arrest are presented below.
