摘要
本文基于“透热转气”理论与膀胱经循行理论，总结1例老年脓毒性休克伴高热患者的中西医结合护理经验。在规范抗感染治疗基础上，创新性采用铜砭刮痧（重点施术于督脉、膀胱经及四井排毒部位，共3次）联合耳尖放血（单次放血量0.5 mL）的中医外治法，同步优化液体复苏策略、个体化营养支持及阶梯式康复训练方案。通过“引营血分之邪热外透气分而解”的核心机制，该疗法显著改善患者持续高热、意识障碍及末梢循环，为老年脓毒症患者的高热管理提供可复制的临床路径。
Abstract
Based on the "Heat Penetration and Qi Transformation" theory and the meridian theory of the Bladder Meridian， this paper summarizes the experience of integrated traditional Chinese and Western nursing for an elderly patient with septic shock and high fever. On the basis of standardized anti-infection treatment， an innovative external therapy of traditional Chinese medicine was adopted， including copper scraping （focusing on the Governor Vessel， Bladder Meridian， and Four Well Detoxification Points， for a total of 3 sessions） combined with auricular point bloodletting （0.5 mL per session）. Simultaneously， the fluid resuscitation strategy， individualized nutritional support， and stepwise rehabilitation training plan were optimized. Through the core mechanism of "guiding the pathogenic heat from the Ying and Xue levels to the Qi level for elimination"， this therapy significantly improved the patient's persistent high fever， consciousness disorder， and peripheral circulation， providing a replicable clinical pathway for the management of high fever in elderly patients with sepsis.
