摘要
本文总结1例慢性阻塞性肺疾病急性加重患者的规范化转运案例。该患者在急诊留观治疗2 d后病情趋于稳定，经家属同意后，医护团队采用SBAR标准化沟通模式联合急诊转运清单，制定个性化转运护理计划：风险评估与资源配置：根据病情分级及转运风险配置人员、药品及仪器，主动预防潜在风险；信息传递与病情管理：通过SBAR模式确保医疗信息准确传递，全程掌握患者病情变化，预防不良事件；多环节优化：与接收科室提前沟通，规划最优转运路线以缩短时间，同时缓解患者焦虑，提升家属满意度；效果评价：最终实现安全转运，为患者生命安全提供有效保障。
Abstract
This article summarizes a case of standardized transport for a patient with acute exacerbation of chronic obstructive pulmonary disease （COPD）. After two days of observation and treatment in the emergency department， the patient's condition stabilized. With family consent， the medical team implemented a personalized transport care plan using the SBAR （Situation-Background-Assessment-Recommendation） standardized communication model combined with an emergency transport checklist. Key interventions included： Risk Assessment and Resource Allocation： Personnel， medications， and equipment were allocated based on disease severity and transport risks to proactively mitigate potential hazards. Information Transfer and Condition Management： The SBAR model ensured accurate medical information exchange， enabling real-time monitoring of the patient’s status and prevention of adverse events. Multidisciplinary Optimization： Pre-transport coordination with the receiving department optimized the route to reduce transit time， alleviated patient anxiety， and improved family satisfaction. Outcome Evaluation： The approach achieved safe transport， effectively safeguarding the patient’s life.
