摘要

本文报告1例85岁高龄女性丹毒合并下肢溃疡患者的中西医结合护理经验。患者因“右下肢红肿疼痛2周“入院，西医诊断为右下肢丹毒伴皮肤溃疡、血栓性静脉炎等，中医辨证为湿热下注证。通过中西医结合干预（抗生素治疗联合萆薢渗湿汤内服、金黄散外敷）及系统化护理（疼痛管理、创面护理、循环改善等），患者感染指标显著改善（CRP由34.79 mg/L降至13.37 mg/L），溃疡面积缩小，疼痛缓解。护理重点包括规范化中药塌渍操作、动态监测溃疡愈合情况及个性化健康指导等。本案例表明，针对高龄丹毒患者，中西医结合护理可有效控制感染、促进创面修复，并为同类患者的护理提供参考。

Abstract

This article presents a case report on the integrated traditional Chinese and Western medicine （TCM-WM） nursing approach for an 85-year-old female patient with erysipelas complicated by lower limb ulcer. The patient was admitted with a two-week history of "redness， swelling， and pain in the right lower limb." Western medical diagnosis confirmed erysipelas with skin ulcer and thrombophlebitis， while TCM pattern differentiation identified dampness-heat pouring downward syndrome. Through combined interventions （antibiotic therapy alongside oral administration of Bixie Shenshi Decoction and topical application of Jinhuang Powder） and systematic nursing care （pain management， wound care， circulation improvement， etc.）， the patient exhibited significant improvement in infection markers （CRP decreased from 34.79 mg/L to 13.37 mg/L）， ulcer size reduction， and pain relief. Key nursing strategies included standardized TCM wet compressing techniques， dynamic monitoring of ulcer healing， and personalized health guidance. This case demonstrates that integrated TCM-WM nursing can effectively control infection， promote wound recovery in elderly erysipelas patients， and serve as a reference for similar cases.
