摘要
本文总结1例采用长效抗菌膜敷料治疗合并多重基础疾病的失禁性皮炎患者的护理实践。患者为长期卧床痴呆状态老年患者，会阴部皮肤护理难度显著。在常规治疗基础上，团队制定个性化护理方案，通过“清洁—保护—修复”三位一体干预策略，即选用长效抗菌膜敷料外喷形成物理屏障，联合标准化失禁护理流程（包括pH平衡清洁、定时评估、体位管理等）。经9 d干预，患者皮炎创面显著改善，舒适度与生活质量明显提升。本案例证实，针对高风险失禁性皮炎患者，长效抗菌材料结合系统化护理可有效促进创面愈合，为临床难治性IAD护理提供可推广的实践范式。
Abstract
This article summarizes the nursing practice of a case of incontinence-associated dermatitis （IAD） treated with long-acting antibacterial film dressing in a patient with multiple underlying diseases. The patient was an elderly individual with long-term bedridden dementia， presenting significant challenges in perineal skin care. Building on conventional therapy， the team developed a personalized nursing plan implementing a "cleansing-protection-repair" triad intervention strategy. This involved the application of long-acting antibacterial film dressing to form a physical barrier， combined with a standardized incontinence care protocol （including pH-balanced cleansing， regular assessment， and posture management）. After 9 days of intervention， the patient's IAD lesions showed marked improvement， with enhanced comfort and quality of life. This case demonstrates that for high-risk IAD patients， long-acting antibacterial materials integrated with systematic nursing can effectively promote wound healing， offering a replicable clinical paradigm for refractory IAD management.
