摘要
本文总结1例术后伤口愈合不良合并2型糖尿病患者应用中药掺药联合红光照射治疗的护理经验。基于“煨脓长肉”理论，采用分期化腐生肌中药掺药技术，联合红光照射及糖尿病饮食管理模式，实施中西医协同干预。治疗38 d后，患者创面面积由10 cm×15 cm缩小至4 cm×8 cm（愈合率78.7%），渗液量由15 mL/d降至1.5 mL/d（下降90%），NRS疼痛评分由6分降至2分。本案例创新性提出“清创—促愈—上皮”三阶段序贯疗法，为糖尿病慢性伤口管理提供临床路径。
Abstract
This paper summarizes the nursing experience of treating a postoperative wound non-healing patient with type 2 diabetes using Chinese herbal medicine dressing combined with red light irradiation. Based on the theory of "encouraging pus formation to promote flesh growth"， the staged debridement and tissue regeneration Chinese herbal medicine dressing technique was adopted， combined with red light irradiation and a diabetic diet management model， to implement a collaborative intervention of traditional Chinese and Western medicine. After 38 days of treatment， the wound area of the patient decreased from 10 cm×15 cm to 4 cm×8 cm （healing rate 78.7%）， the exudate volume decreased from 15 mL/d to 1.5 mL/d （a 90% reduction）， and the NRS pain score decreased from 6 to 2. This case innovatively proposed a three-stage sequential therapy of "debridement - promoting healing - epithelialization"， providing a clinical pathway for the management of diabetic chronic wounds.
