摘要

维持性血液透析患者突发严重心律失常虽极为罕见，但病情凶险、病死率高。本病例报道1例40岁女性维持性血液透析患者，在透析过程中及结束后短时间内连续发生2次心室颤动。经迅速启动规范化心肺复苏（CPR）并实施电除颤紧急抢救，多学科协作完成危重患者床边转运，后续植入式心脏复律除颤器（ICD）治疗，并配合中西医结合综合护理干预，患者最终抢救成功，病情稳定后顺利出院。本案例凸显了快速有效的急救处置流程、多学科协作模式及个体化综合护理干预在危重血液透析患者心律失常救治中的关键作用，为临床同类病例的救治提供了重要实践参考。

Abstract

Although sudden severe arrhythmia in maintenance hemodialysis patients is extremely rare， it is a life-threatening condition with a high mortality rate. This case report describes a 40-year-old female maintenance hemodialysis patient who experienced two consecutive episodes of ventricular fibrillation during and shortly after hemodialysis. The patient was successfully resuscitated through prompt initiation of standardized cardiopulmonary resuscitation （CPR） and emergency defibrillation， with multidisciplinary collaboration for critical patient bedside transfer. Subsequently， an implantable cardioverter defibrillator （ICD） was implanted， and comprehensive integrated traditional Chinese and Western medicine nursing intervention was provided. The patient was eventually rescued successfully and discharged in a stable condition. This case highlights the critical role of rapid and effective emergency response procedures， multidisciplinary collaboration， and individualized comprehensive nursing intervention in the treatment of arrhythmia in critically ill hemodialysis patients， providing an important practical reference for similar clinical cases.
