摘要

本案例针对1例合并多种严重系统性疾病的高龄重度特应性皮炎（AD）患者，在全身用药风险极高的困境下，实施以中药溻渍（冰柏液）外治为核心，结合内服清热利湿中药、五音疗法调畅情志、辨证施膳及皮肤屏障护理的中西医结合立体化干预方案。经10 d干预，患者特应性皮炎评分（SCORAD）由68.5分降至30.2分，瘙痒视觉模拟评分（VAS）由8分降至2分，匹兹堡睡眠质量指数（PSQI）总分由18分降至9分，焦虑情绪显著缓解。该方案在规避系统用药风险的同时，有效控制皮损症状并改善身心状态，体现了中医“外治祛邪、内调固本、身心共治”的护理优势，为复杂共病AD患者提供了安全可行的临床管理路径。

Abstract

This case report describes a comprehensive integrative intervention for an elderly patient with severe atopic dermatitis （AD） and multiple systemic comorbidities， where systemic medication posed high risks. The treatment protocol centered on external herbal soaking therapy （using Bingbai solution） combined with oral heat-clearing and dampness-resolving herbs， five-element music therapy for emotional regulation， syndrome-differentiated dietary guidance， and skin barrier rehabilitation. After 10 days of intervention， the patient's SCORAD score decreased from 68.5 to 30.2， itch VAS score dropped from 8 to 2， and PSQI total score improved from 18 to 9， with marked relief of anxiety. This approach effectively controlled skin lesions and enhanced physical-mental well-being while avoiding systemic drug risks， demonstrating the TCM nursing principles of "external therapy to eliminate pathogens， internal regulation to consolidate constitution， and holistic mind-body care." It provides a safe and feasible clinical pathway for managing complex multimorbid AD.
