摘要

本文报道1例50岁女性面部黑变病患者的中医外治护理经验。该患者面部曝光部位存在20余年弥漫性灰褐色、紫褐色网状斑片，针对西医治疗局限性，采用雷火灸联合刮痧纯中医外治法，结合情志疏导、健康宣教及并发症预防系统化护理。通过黄褐斑面积和严重程度指数（MASI）评分评估，患者初始评分为15.45分（中度），经1年余中医外治调理后，面部色素显著改善，皮肤质地恢复良好，MASI评分降至6.30分（轻度），且全程无不良反应。研究表明，刮痧联合雷火灸配合整体护理安全有效，可弥补西医治疗不足，为顽固性面部黑变病提供有价值的临床实操参考。

Abstract

This paper reports the traditional Chinese medicine （TCM） external treatment and nursing experience for one 50-year-old female patient with facial melanosis. The patient presented with diffuse gray-brown and purplish-brown reticular macules on the photo-exposed areas of the face for over 20 years. In view of the limitations of Western medical treatment， pure TCM external therapies of thunder-fire moxibustion combined with scraping therapy were adopted， along with systematic nursing interventions including emotional counseling， health education， and prevention of complications. Evaluated by the Melasma Area and Severity Index （MASI）， the patient had an initial score of 15.45 （moderate）， which decreased to 6.30 （mild） after more than one year of TCM external conditioning， with significant improvement in facial pigmentation and restoration of skin texture. No adverse reactions occurred throughout the treatment. The findings indicate that scraping therapy combined with thunder-fire moxibustion alongside holistic nursing care is safe and effective， which can compensate for the shortcomings of Western medical treatment and provide a valuable practical reference for the clinical management of refractory facial melanosis.
