摘要

本文总结1例85岁脑卒中后肢体偏瘫患者的护理经验。在常规治疗基础上，采用中药塌渍联合红外线外治，并针对性落实皮肤感觉评估与烫伤预防、阶梯式肢体康复训练、情志调护及延续性指导等中西医结合护理措施。干预2周后，患者左侧肢体肌力由3级提升至4级，Barthel指数由50分提高至70分，汉密尔顿焦虑量表（HAMA）评分由23分降至13分，无护理并发症发生。该联合疗法能有效促进局部血液循环与神经功能恢复，配合精准的护理风险管控，可显著提升高龄偏瘫患者的康复质量，且操作简便，值得在临床实践中进一步探索和推广。

Abstract

This article summarizes the nursing experience of caring for an 85-year-old patient with post-stroke hemiplegia. On the basis of conventional treatment， a combined external therapeutic approach integrating Chinese herbal soaking with infrared radiation was employed， complemented by targeted integrated Chinese and Western medicine nursing interventions， including skin sensation assessment and burn prevention， staged limb rehabilitation training， emotional regulation， and continuity-of-care guidance. After two weeks of intervention， the muscle strength of the patient's left limb improved from grade 3 to grade 4， the Barthel Index score increased from 50 to 70， and the Hamilton Anxiety Scale （HAMA） score decreased from 23 to 13， with no nursing-related complications observed. This combined therapy can effectively promote local blood circulation and neurological function recovery. Combined with precise nursing risk management， it significantly enhances the rehabilitation quality of elderly hemiplegic patients. Moreover， the intervention is easy to perform and merits further exploration and promotion in clinical practice.
