摘要

本文总结1例高龄肺肾两虚型哮病合并顽固性失眠患者的护理过程。患者前期予耳穴疗法干预，咳喘及睡眠症状改善不明显。经评估后，医护人员基于“金水相生”理论，以“补肺益肾、宁心安神”为治则，调整干预方案，即以头面部推拿联合头部刮痧为核心中医外治技术，同步配合辨证施护、情志疏导、饮食调护及呼吸功能锻炼。干预后，患者咳喘发作频率降低、夜间憋醒次数减少，入睡困难、夜间易醒等失眠症状明显改善，睡眠时长及日间精神状态好转，生活质量显著提升。本案例提示，基于“金水相生”理论指导的头面部推拿联合头部刮痧，可通过补肺益肾、安神助眠，对肺肾两虚型哮病伴顽固性失眠患者具有良好的护理干预效果，可为临床同类病例的中西医结合护理提供参考。

Abstract

This paper summarizes the nursing process for an elderly patient with asthma of lung-kidney dual deficiency pattern complicated by intractable insomnia. The patient initially received auricular acupoint therapy； however， the symptoms of cough， wheezing， and sleep disturbance showed no obvious improvement. Following comprehensive reassessment， the medical and nursing team adjusted the intervention plan based on the traditional Chinese medicine （TCM） theory of "metal-water mutual generation" （the mutual promoting relationship between the lung and kidney）， adopting the therapeutic principle of "tonifying the lung and kidney， and tranquilizing the mind." Head and facial massage combined with scalp scraping were employed as the core TCM external treatment techniques， accompanied by syndrome-differentiated nursing， emotional regulation， dietary guidance， and respiratory function exercise. After the intervention， the frequency of cough and wheezing episodes was reduced， nighttime episodes of dyspnea awakening decreased， and insomnia symptoms including difficulty falling asleep and easy awakening were markedly alleviated. Sleep duration and daytime energy improved， with a significant enhancement in quality of life. This case suggests that head and facial massage combined with scalp scraping， guided by the "metal-water mutual generation" theory， exerts favorable nursing effects on elderly patients with lung-kidney dual deficiency asthma complicated by intractable insomnia through tonifying the lung and kidney as well as calming the mind and promoting sleep. It may serve as a reference for integrated traditional Chinese and Western medicine nursing care of similar clinical cases.
