基于吴尚先中药外治思想指导下中药溻渍联合穴位贴敷干预膝骨关节病患者的个案护理
摘要
本文总结1例老年血瘀气滞型膝骨关节病患者的护理经验，以吴尚先“外治之理即内治之理，外治之药即内治之药”的外治思想为指导，实施中药溻渍联合穴位贴敷的中医特色护理，并配合病情观察、基础病管理等综合措施。护理遵循活血舒筋、筋骨并治，溻渍覆经及贴敷定点的原则，靶向干预血瘀气滞、经络阻滞的核心病机。干预后患者膝关节疼痛缓解，关节功能改善，日常生活能力提高，住院期间无跌倒等不良事件发生。提示基于吴尚先外治思想的中药溻渍联合穴位贴敷及中医综合护理用于血瘀气滞证膝骨关节病安全、有效、可行，可为临床老年膝痹病的中医特色护理提供实践参考。
Abstract
This paper summarizes the nursing experience of an elderly patient with knee osteoarthritis of the blood stasis and qi stagnation pattern. Guided by Wu Shangxian's external treatment philosophy that "the principle of external treatment is identical to that of internal treatment， and the medicines used in external treatment are the same as those used in internal treatment，" TCM characteristic nursing interventions were implemented， including medicinal wet compress combined with acupoint application， along with comprehensive measures such as condition monitoring and management of underlying diseases. The nursing care adhered to the principles of promoting blood circulation and relaxing tendons， treating both tendons and bones simultaneously， applying wet compress along meridians， and targeting acupoint application， with the aim of addressing the core pathogenesis of blood stasis， qi stagnation， and meridian obstruction. Following the intervention， the patient's knee joint pain was relieved， joint function was improved， and the ability to perform activities of daily living was enhanced. No adverse events such as falls occurred during hospitalization. The findings suggest that medicinal wet compress combined with acupoint application and comprehensive TCM nursing care， grounded in Wu Shangxian's external treatment theory， is safe， effective， and feasible for knee osteoarthritis of the blood stasis and qi stagnation pattern， and may provide practical reference for TCM characteristic nursing care of elderly patients with knee impediment disease in clinical practice.
