摘要

总结1例75岁膀胱填塞术后继发心、肾、呼吸及消化等多器官功能障碍患者的中西医结合护理经验。西医层面实施持续膀胱冲洗、CRRT容量管理、机械通气、多管路维护及营养支持；中医层面紧扣“气不摄血”至“阳气衰微”的病机演变，分期应用耳穴压豆、穴位贴敷、中药内服及辨证施护。经28 d救治与护理，患者成功脱机拔管，多器官功能显著改善。中西医结合护理对高龄多器官功能障碍患者具有协同增效作用。

Abstract

This paper summarizes the nursing experience of integrated traditional Chinese and Western medicine for a 75-year-old patient with secondary multiple organ dysfunction （involving the heart， kidney， respiratory， and digestive systems） following surgery for bladder tamponade. From the Western medicine perspective， continuous bladder irrigation， continuous renal replacement therapy （CRRT） volume management， mechanical ventilation， multi-lumen catheter maintenance， and nutritional support were implemented. From the Traditional Chinese Medicine （TCM） perspective， nursing interventions were closely aligned with the disease mechanism evolution from "Qi failing to control blood" to "Yang Qi exhaustion and decline，" and were administered in different stages， including auricular bean pressing， acupoint application， oral administration of Chinese herbal medicine， and nursing care based on syndrome differentiation. Following 28 days of treatment and nursing care， the patient was successfully weaned from mechanical ventilation and extubated， with significant improvement in multiple organ function. The integrated traditional Chinese and Western medicine nursing care demonstrates synergistic effects for elderly patients with multiple organ dysfunction.
