摘要

本研究探讨穴位按摩联合中药外敷在腰椎间盘突出症疼痛管理中的应用效果。对1例伴慢性疼痛患者实施个体化穴位按摩、中药外敷，配合疼痛监测、情志护理及康复教育等综合护理，干预14 d。结果显示，视觉模拟评分法（VAS）评分由7分降至2分，Barthel指数由75分升至100分，下肢放射性疼痛及麻木感基本消失，腰椎活动度明显改善，3个月随访无复发。该方案安全有效、操作简便，能疏通经络、活血化瘀，配合系统化中医护理可巩固疗效、降低复发率，适合基层医院推广应用。

Abstract

This study investigates the clinical effects of acupoint massage combined with traditional Chinese medicine （TCM） external application in the pain management of lumbar disc herniation （LDH）. A patient with chronic pain received individualized acupoint massage and TCM external application， supplemented by comprehensive nursing interventions including pain monitoring， emotional care， and rehabilitation education， with an intervention duration of 14 days. The results demonstrated that the Visual Analogue Scale （VAS） score decreased from 7 to 2， the Barthel Index increased from 75 to 100， radicular pain and numbness in the lower extremities were essentially alleviated， and lumbar range of motion was significantly improved. No recurrence was observed at the 3-month follow-up. This protocol is safe， effective， and easy to perform. By dredging the meridians， promoting blood circulation， and resolving blood stasis， and when integrated with systematic TCM nursing care， it can consolidate therapeutic outcomes and reduce the recurrence rate， making it suitable for promotion and application in primary healthcare settings.
