摘要

多病共存并发心力衰竭的临床管理难度大，本例患者合并冠心病、高血压3级及2型糖尿病，治疗矛盾突出。本文总结其中西医结合护理经验，采用出入量监测联合中药熏蒸、耳穴压贴等精细化容量管理措施，配合个体化饮食干预及五行音乐疗法等情志调护方案。经14 d干预，患者水肿消退、BNP水平显著下降且胸闷憋气等症状改善。本案例为多病共存心力衰竭患者的中西医结合护理提供了实践参考。

Abstract

The clinical management of heart failure complicated by multiple comorbidities presents considerable challenges. In this case， the patient presented with concurrent coronary heart disease， grade 3 hypertension， and type 2 diabetes mellitus， with prominent therapeutic conflicts among the conditions. This paper summarizes the integrated Chinese and Western medicine nursing experience for this case. Refined volume management strategies were implemented， including intake and output monitoring combined with traditional Chinese medicine （TCM） fumigation therapy and auricular acupressure. Additionally， individualized dietary intervention and emotional regulation measures such as five-element music therapy were employed. Following a 14-day intervention， the patient's edema resolved， B-type natriuretic peptide （BNP） levels decreased significantly， and symptoms including chest tightness and dyspnea were alleviated. This case provides practical reference for integrated Chinese and Western medicine nursing care in patients with heart failure and multiple comorbidities.
