摘要

本文总结1例基于十二经络盛衰择时腧穴艾灸序贯穴位贴敷治疗多系统萎缩合并尿潴留患者的护理过程。该患者因多系统萎缩并发尿潴留，院外反复留置尿管、行间歇导尿治疗，合并泌尿系统感染。经完善膀胱超声检查，中西医结合诊断为尿潴留（肾阳衰微，气化无权）。护理过程中实行“因时而护，择时而治”，选取针对性穴位和经络实施艾灸联合穴位贴敷的中医特色护理技术干预，同时配合五音疗法及生活护理计划与措施。经干预后，患者尿潴留症状显著改善，随访1个月未再出现尿潴留症状，泌尿系统感染发生率降低，生活质量与生存质量得到提升，焦虑情绪得以缓解，有助于促进患者机体功能康复。

Abstract

This article summarizes the nursing care process for a patient with multiple system atrophy （MSA） complicated by urinary retention， who was treated with time-selected acupoint moxibustion based on the waxing and waning of the twelve regular meridians combined with sequential acupoint application. The patient had experienced repeated indwelling catheterization and intermittent catheterization outside the hospital due to MSA-associated urinary retention， accompanied by recurrent urinary tract infection. Following comprehensive bladder ultrasonography and an integrated diagnostic approach combining traditional Chinese medicine （TCM） and Western medicine， the condition was identified as urinary retention attributed to kidney yang depletion with impaired qi transformation. During the nursing process， the principle of "caring in accordance with time and treating at the appropriate moment" was implemented. Targeted acupoints and meridians were selected for moxibustion combined with acupoint application—a characteristic TCM nursing intervention—together with five-element （pentatonic） music therapy and individualized daily care planning and measures. After intervention， the patient's urinary retention symptoms improved significantly. During a one-month follow-up， no recurrence of urinary retention was observed， the incidence of urinary tract infection decreased， quality of life and health-related quality of life were enhanced， anxiety was alleviated， and overall functional rehabilitation was promoted.
