摘要

本文总结1例慢性肾脏病5期腹膜透析合并重度水肿（阳虚水泛证）患者的中西医结合护理经验。患者存在多重基础疾病叠加、重度肥胖合并容量负荷过重、左侧肢体活动不利及中医阳虚水泛证等特殊性，通过实施系统化护理评估、精准化护理诊断与计划、个体化护理措施及效果评价，并结合中医辨证施护，采用真武汤合五皮饮内服及中药塌渍、隔物灸、耳穴疗法等特色护理技术，患者水肿显著消退，心功能改善，营养状况提升，顺利出院。

Abstract

This article summarizes the nursing experience of integrated Chinese and Western medicine applied to a patient with stage 5 chronic kidney disease undergoing peritoneal dialysis complicated with severe edema （pattern of yang deficiency with water flooding）. The patient presented with a constellation of complex clinical conditions， including multiple comorbid underlying diseases， severe obesity with volume overload， impaired mobility of the left limb， and yang deficiency with water flooding pattern in traditional Chinese medicine. Through the implementation of systematic nursing assessment， precise nursing diagnosis and planning， individualized nursing interventions， and outcome evaluation， combined with syndrome differentiation-based traditional Chinese medicine nursing care—encompassing the oral administration of Zhenwu Decoction combined with Wupi Decoction and characteristic nursing techniques such as traditional Chinese medicine hot compress application （Tazi therapy）， indirect moxibustion， and auricular point therapy—the patient's edema was significantly alleviated， cardiac function was improved， and nutritional status was enhanced， enabling a successful discharge from the hospital.
