摘要

本文总结1例成人左冠状动脉异常起源于肺动脉（ALCAPA）合并重度二尖瓣关闭不全、三尖瓣关闭不全及前降支长段心肌桥患者，接受多术式联合外科手术的围手术期护理经验。该病例病情复杂危重，护理重点在于术前予五联抗心衰治疗以改善心功能，并同步给予心理支持，为手术实施奠定基础；术后通过循环精细化管理、呼吸系统并发症防控及抗凝安全管理，配合贯穿全程的心脏康复干预，保障手术成功并降低死亡率。患者于术后第7天顺利康复出院。

Abstract

This article summarizes the perioperative nursing experience of an adult patient with anomalous left coronary artery from the pulmonary artery （ALCAPA） complicated by severe mitral regurgitation， tricuspid regurgitation， and a long-segment myocardial bridge in the left anterior descending artery， who underwent combined surgical procedures. Given the complex and critical condition of the patient， preoperative care focused on quintuple anti-heart-failure therapy to improve cardiac function， along with simultaneous psychological support， thereby laying the foundation for surgical intervention. Postoperative management involved precise hemodynamic monitoring， prevention and control of respiratory complications， and anticoagulation safety， complemented by cardiac rehabilitation interventions implemented throughout the entire course of treatment， which ensured surgical success and reduced mortality. The patient recovered smoothly and was discharged on postoperative day seven.
