摘要

目的

总结中医穴位放血疗法联合中西医结合护理在外感高热患者中的应用效果，为急症中医护理提供参考。

方法

回顾性分析2025年12月收治的1例外感高热（风热犯表证）患者的临床资料，该患者以“发热2 d，最高体温39℃”为主诉，特殊性在于其为72岁高龄患者，发热峰值高，伴随关节、咽喉及头面等多部位症状，且自行口服清肺消炎丸、玛巴洛沙韦治疗无效，对护理评估与干预的精细度提出了更高要求。在对症治疗基础上，于就诊当日10：30实施耳尖穴点刺放血1次，并配合辨证施护、病情监测及健康教育。

结果

干预后1 h，患者体温由39℃降至37.6℃，关节酸痛、咽痛、咳嗽、头痛、牙痛及耳痛等风热犯表伴随症状明显缓解；干预后24 h体温维持在正常范围，症状消失。随访1周无复发，离院后24 h采用Likert 5级评分法测评患者满意度，得分为9.8分（满分10分）。

结论

中医穴位放血疗法操作简便、退热迅速，配合系统化护理干预，能有效提升外感高热患者的治疗效果与舒适度，值得临床推广。

Abstract

Objective

To summarize the application effects of Traditional Chinese Medicine （TCM） acupoint bloodletting therapy combined with integrative Chinese and Western medicine nursing in patients with exogenous high fever， and to provide a reference for TCM emergency nursing.

Methods

Clinical data of a patient with exogenous high fever （wind-heat attacking the exterior pattern） admitted in December 2025 were retrospectively analyzed. The patient presented with a chief complaint of "fever for 2 days， with a peak temperature of 39℃." The particularity of this patient was that she was 72 years old， with a high fever peak accompanied by symptoms involving multiple sites including the joints， throat， and head and face. In addition， self-administered oral Qingfei Xiaoyan Pills and baloxavir marboxil proved ineffective， placing higher demands on the precision of nursing assessment and intervention. On the basis of symptomatic treatment， bloodletting pricking at the Erjian （apex of ear） acupoint was performed once at 10：30 on the day of consultation， in conjunction with nursing care based on pattern differentiation， condition monitoring， and health education.

Results

One hour after the intervention， the patient's body temperature decreased from 39℃ to 37.6℃， and the accompanying symptoms of wind-heat attacking the exterior pattern， including joint soreness and pain， sore throat， cough， headache， toothache， and ear pain， were significantly relieved. Twenty-four hours after the intervention， the body temperature remained within the normal range and the symptoms disappeared. No recurrence was observed during a one-week follow-up. Patient satisfaction was evaluated using a 5-point Likert scale at 24 hours after discharge， yielding a score of 9.8 （out of 10）.

Conclusion

TCM acupoint bloodletting therapy， characterized by simple operation and rapid antipyretic effect， combined with systematic nursing interventions， can effectively enhance the therapeutic outcomes and comfort of patients with exogenous high fever， and is worthy of clinical promotion.
